When you call for your appointment details about your visit, including your insurance coverage and what to expect, will be discussed with you. You will be told what time you need to show up for your appointment and you may be instructed to come in earlier in order to complete paperwork.
If you are a new patient or are being evaluated for a new problem you can save time by logging into your Electronic Medical Record(EMR) or creating a login on the EMR patient portal through the ENT-Cloud link on this page. You can update your history there or print and fill out the forms from these three links and bringing it with you. Otherwise, you will be asked to fill them out after you arrive.
